Parliamentary and Health Service Ombudsman Julie Mellor said:
Many people complain to us because they don’t want someone else to endure what they or their loved one went through.
'We are pleased that our casework has been useful to the committee on this important issue, to enable them to make important recommendations so that avoidable suffering does not continue.
'We are still seeing too many tragic cases where people are suffering unnecessarily because they are not receiving the right care or treatment at the end of their lives.’
