Joint letter to DHSC from the Ombudsman and Patient Safety Commissioner

Ombudsman Rob Behrens and the Patient Safety Commissioner wrote to the Permanent Secretary of DHSC to convey their joint concerns on the confrontational culture created by the complaints process in areas of the NHS that undermines patient safety.
