Parliamentary and Health Service Ombudsman Julie Mellor said:
Our casework shows that too many people are dying without dignity. We shared the insight from our casework so that healthcare leaders can consider how to improve end of life care, to help prevent similar cases from happening again.
'We welcome the draft guidance, in particular the emphasis on individualised care and decision making being shared between the professional and their patient.'
